Summary
Description
Responsible for providing day to day assistance and support to Community Inclusion Program consumers by monitoring and supervising consumers, assisting with transition times, assisting with task and skill training, implementing ISP objectives and providing personal care.
1. Assists with the one-on-one task training of clients in accordance with written ISP goals and objectives.
2. Monitors and documents individual client progress toward written ISP goals and objectives.
3. Maintains daily schedule of training activities.
4. Collects daily data and assists in the development of client goals and objectives.
5. Informs Program Services Specialists/Program Manager of client’s achievements, incidents, progress, or problems as needed.
6. Assists in the creation and facilitation of specialized training.
7. Performs morning and afternoon transition duties including but not limited to transportation assistance, personal care, and classroom activities.
8. Reports monthly on ISP progress (Progress Notes).
9. Provides one-on-one specialized assistance to clients with identified needs for additional support.
10. Provides Human Rights and serves as an advocate for clients.
Education and Requirements:
High School Diploma/GED plus one (1) year of work experience required.
Skills and Abilities:
The Program Services Assistant is expected to work cooperatively with his/her supervisor, and collaboratively with fellow employees, and consumers of service. The PSA must have the skills and abilities to:
· Present a positive and professional image to public, coworkers and Didlake consumers
· Have a valid driver’s license
· Be insurable under Didlake’s automobile policy
· Be willing to use their vehicle for travel.
· Be deemed eligible for hire by DMHMRSAS in accordance with the criminal registry background check.
